EXPERIENCED RN INPATIENT CASE MANAGER NEEDED FOR PRESTIGIOUS HEALTHCARE ORGANIZATION IN BEAUTIFUL SAN FRANCISCO!!!

The successful applicant will be responsible for care coordination and discharge planning for a specific population of healthplan members. You will be conducting onsite and telephonic assessments within the hospital network, coordination of an appropriated plan of care, working collaboratively with the member and his or her family, as well as health plan representatives, physician network, and home health care providers, for the purpose of coordinating care to meet the patient's overall care requirements. Additionally, the RN Case Manager facilitates the authorization of outpatient services. The position assures that authorizations meet payer requirements, follow appropriate policies and procedures and are medically necessary, utilizing approved guidelines.
