Nurse Practitioner to work in the capacity of evaluating our senior members then coordinating the findings to the PCP.
Responsibilities:
Provides annual health assessment to senior members
Documents appropriate diagnoses and medications
Updates the members PCP
Refers member to appropriate disease management programs as applicable.
May provide in-home assessments

Qualifications
Good interpersonal skills
Excellent communication skills
Ability to relate to senior patients and their families
Ability to perform complete and accurate patient assessments

Education and Experience
Completion of Nurse Practitioner program
Minimum 5 years experience- preferably with senior adults
Bilingual Spanish a plus
Knowledge in diagnostic coding a plus
